At MyMedMart we understand that dealing with insurance can be difficult and confusing. We will submit claims to Medicare and your private insurance carrier for you.
HOW DOES THE PROCESS WORK?
1. Complete insurance form.
2. Select products to be purchased.
3. Proceed to checkout, purchase with credit card.
4. Obtain a dispensing form from physician.
A. You can obtain dispensing order from your physician then fax it to (515) 832-4851,
or mail it to: 723 Seneca St., Webster City, IA 50595.
OR
B. For faster service have your physician call in a dispensing order to: 1-888-832-7545.
5. Once a dispensing order is received, your order will be shipped, and billed to your credit card.
6. For Medicare beneficiaries certain items require a detailed prescription prior to delivery.
Have your physician complete the detailed prescription form and mail to: 717 Seneca St., PO Box 215, Webster City, IA 50595 or fax to (515) 832-4851
7. My MedMart will then obtain any additional information needed from your physician.
8. Do not submit claims directly to Medicare. As a Medicare provider we are obligated to submit claims to Medicare for you, and we are happy to do so, but Medicare will make the final determination on benefits and will reimburse you directly.
9. The submission of your claim to Medicare or your private insurance company does not guarantee payment of that claim. Please refer to your coverage guidelines or contact your insurance company for specific coverage information.
